Missed Appointment Policy:
We ask that appointments be cancelled with 48 hours notice. For
nuclear testing if not canceled within 24 hours, patient will be
billed for the price of the medication.
Payment/Insurance Policy:
Please bring your insurance cards to all visits.
Please remember to arrange for referrals as required by your insurance
plan.
Co-pays are due at time of service.
Rx Policy: Prescription refill policy
Prescription Renewal is an important part of
your 0n-going medical care and as such that you plan your needs
according to the following guidelines:
- Prescription refill requests will be called
in to your local pharmacy by 12:00 (noon) of the next business
day.
- Written prescriptions require 72 hours notice
and be either mailed to your home or picked up in person. They
will not be mailed to either your pharmacy or to your mail order
company.
- We do not fax written prescriptions.
- The on-call physician cannot refill
your prescriptions evenings, weekends or holiday. The physician
does not have access to your medical records.

We participate with most major insurance plans. If your insurance
is not listed below, please contact our office for further information.
